POOL CASE MANAGER RN OPPORTUNITIES Per Diem Opportunities available within our following locations:
At Baptist Health, we have an integrated collaborative Triad Model with specialists in Utilization Review, Social Work Case Management and RN Case Management to deliver comprehensive and patient centered care across the continuum.
The Case Manager RN supports the physician and interdisciplinary team in facilitating patient care, with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction while managing the cost of care. The role integrates and coordinates care facilitation and discharge planning functions while working in partnership with the UR RN and Social Work Case Manager, and is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of stay (LOS), and promote efficient utilization of resources. Specific functions within this role include: facilitation of the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement; application of process improvement methodologies in evaluating outcomes of care; coordinating communication with the healthcare team, patients, families and post-acute care services; and readmission screening and prevention.
Florida licensed Registered Nurse.
Associates Degree in Nursing required; BSN strongly preferred.
Excellent interpersonal communication and negotiation skills, strong analytical, data management and computer skills
CCM Case Management Administrator Certification, NACCM Care Manager Certified, ABMCM Certified Managed Care Nurse, RNCB Certified Rehabilitation Registered Nurse, ANCC Nursing Case Management,, CCMC Case Manager, ACMA ACM Certification, NBCC Certification in Continuity of Care, Advanced, CDMS Certified Disability Management Specialist.
Minimum 3 years hospital clinical experience with one to three years of hospital or payor case management experience preferred
Current working knowledge of discharge planning, utilization management, case management and performance improvement preferred. Understanding of pre-acute and post-acute venues of care and post-community resources preferred
