DESCRIPTION: The Palliative Care Social Worker is responsible for the delivery of highly skilled counselling and case management services to patients with advanced disease about their illness, treatment processes/options and supportive interventions. The PCSW provides psychosocial assessment, health education to patients and their families/caregivers around coping skills and education on nonpharmacological symptom and management strategies. Working In conjunction with the Palliative Care MD and Nurse Practitioner Program Coordinator, the PCSW is responsible for planning activities required to achieve a safe discharge plan, coordination of care, identifying and completing applications for required ongoing financial support and helping patients/families navigate complex healthcare related systems. The PCSW is responsible for facilitating advanced care planning discussions, intervening in crises and mediating conflicts within families. REQUIREMENTS: Minimum Education: Master of Social Work Minimum professional experience: 1 year current relevant social work experience Current New Jersey Social Work license-LCSW preferred Excellent verbal and written communication skills. Bilingual-Spanish. Able to work productively in a team structure. Capable of working with multiple tasks simultaneously. Critical thinking and problems solving skills. Self-Motivation. Assertiveness. Negotiating skills.
