Assesses human responses to disease process, illness, and the death and dying process. Identifies health care needs for the patient and their families to provide education and an environment to assist the patient to a comfortable and dignified death. As needed, will provide restorative, supportive and preventative nursing measures to individuals in their home by executing the medical regimen prescribed by a licensed physician.
Fully responsible for all actions as a licensed nurse and is accountable to clients for the quality of care delivered. Reports to his/her designated immediate supervisor regarding work planning and assignments.
Reports to the Hospice Coordinator.
MAJOR DUTIES AND RESPONSIBILITIES:
*1. Assesses patients by collecting and analyzing data to recognize patients' health problems and perceptions. Interviews patients and/or caregivers to elicit all pertinent information.
*2. Plans nursing intervention designed to teach patients and their caregivers and care management to primarily assist the client to a comfortable and dignified death and dying process. Supports families and caregivers with coping mechanisms and community resources when dealing with the illness, dying and bereavement processes.
*3. Implements, reviews and revises a plan of care and nursing interventions to achieve desired outcomes. Monitors patients after implementation of care plan to assess effectiveness of plan and patient response. Instructs all those involved in delivering care to ensure understanding of education provided. Directs and supervises other levels of clinical personnel in delivery of care to their primary patients (LPN, HCA) as needed. Notifies physician(s) of lab reports and significant changes in patients' conditions as warranted.
*4. Makes appropriate referrals to other disciplines and other resources and coordinates overall care. Works closely with the Medical Social Worker, Bereavement Coordinator and Home Care Aides. Offers community resources to families and caregivers.
*5. Maintains appropriate professional licensure at all time.
*6. Keeps comprehensive, problem-oriented medical records of his/her work as required.
*7. Provides written and statistical reports or assignments and submits these as required.
*8. Assumes responsibility for the recertification of a medical plan of treatment and submits a summarized report to the physician when needed for review and signature. Understands and implements Hospice benefit election periods for the Hospice client and maintains recertifications in conjunction with the election periods.
*9. Collaborates and effectively communicates verbally and in writing with peers, patients, supervisors, physicians and other professionals. Maintains good working relationship with peers, supervisors, referral sources and co-workers. Accepts people in a non-judgmental manner.
*10. Plans, coordinates and guides students, new staff, and other health care team members who are assigned to work with the team.
*11. Reports changes in patient status to supervisor as needed. Seeks and uses clinical guidelines. Participates in team/case conferences and Interdisciplinary Team Meetings as required.
*12. Functions as committee member when requested and contributes professional expertise at assigned committees to provide continuous performance improvement. Participates in community health programs.
*13. Maintains confidentiality regarding information on patients, staff and agency matters.
*14. Assumes the legal responsibility of competency within the scope of the practice of professional nursing.
*15. Distributes and uses patients' supplies prudently with primary car.
*16. Performs other duties as required or assigned by emergency or other operational reasons for which the employee is qualified to perform.

EDUCATION AND/OR EXPERIENCE:
Graduate of an accredited school of nursing required.
Current license to practice as a registered nurse in Pennsylvania required.
Minimum of 2 years clinical experience required and one year of hospice or home health experience preferred. Chemotherapy experience and/or certification preferred.
Must hold a current Pennsylvania driver's license and have a vehicle available for work.
Requires HBV and TB vaccinations.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Must be available to work on call on a rotating basis. This position involves occupational exposure to disease and illness including, but not limited to, Hepatitis B and HIV viruses.
