This position requires that you have experience as a Certified Diabetes Educator and running a Diabetes Self-Management Education Program. Must have been through the entire Accreditation process for DSME, either through ADA or AADE and completed the CDC approved Diabetes Prevention Program course. Must have case managed a client through the DPP program. Must have experience in Program management and Chronic Disease Management. Must have experience in Facilitating Behavior Change and clinical services.
This professional position is responsible for establishing, executing, performing and administrating the Diabetes Services Program for the FL DOH-Flagler CHD per national guidelines such as the Diabetes Education Accreditation Program. This position reports directly to the Nursing Director and adheres to the Department of Health policies and procedures. This position requires advanced knowledge of diabetes, diabetes case management and principles of age and culture specific learning methods.
